If you remove the SD card while in the shell it must do a reset/reboot. Firmware is working this issue.

the issue should be fixed at MNW2_IFWI_X64_D_2014_05_29_1410_SecEnabled.bin drop, currenly we don't need reset/reboot after removing SD card while in the shell, we can type "reconnect -r" to do.

Before reading Tim's latest comment I tried this and types "map -r" that took quite a while as the various commands timed out, but it did return to the shell. This appears to be resolved, marking as such. Thanks Tim.
